Services

Support Coordination for the following LA Office of Citizens with Developmental Disabilities (OCDD) programs:

  • EarlySteps: Ages 0-3. EarlySteps provides services to families with infants and toddlers aged birth to three years (36 months) who have a medical condition likely to result in a developmental delay, or who have developmental delays. This program offers services where the child learns best, in their home or a community setting such as a daycare etc.  Services such as special instruction, various therapies, audiology and vision. social work, family training, autism screenings etc. are offered through this program.   To learn more about EarlySteps, click here.   • Children’s Choice Waiver (CC):.  Through age 21.  Provides support to children living at home.  Includes many services such as family supports, center based respite,  and adaptive modifications and equipment, various specialized therapies and applied behavioral analysis.
  • Supports Waiver: 18 and older. includes services such as supported employment, day habilitation, permanent supportive housing stabilization and transition, and personal emergency response systems.
  • Residential Options Waiver (ROW): Individuals of all ages.  offers services to move from ICF/DD and nursing facilities to community-based settings.  includes residential services, vocational services (e.g. host home and shared living) vocational services (day habilitation and supported employment) and professional services/therapy.
  • New Opportunities Waiver (NOW): Ages 3 and older.  A comprehensive program offering individual and family supports (day and night), community integration and development, environmental and adaptive modifications and equipment, supportive living, day habilitation, supported employment, transportation, professional services, skilled nursing services, center based respite, housing stabilization / transitions services among other services.

Support Coordination for the following Office of Aging and Adult Services (OAAS) programs:

  • Community Choices Waiver (CCW): Adults, age 22 and older  and elderly who are considered disabled according to Medicaid  Standards. The individual must meet  Medicaid  financial  requirements and Nursing Facility Level of Care  Requirements. Supports individuals who choose to live in the community instead of Nursing facilities.   Provides personal care services, personal alert systems, transition from nursing home to the community services,
  • Adult Day Health Care Waiver (ADHC): Adults, age 22 and older and elderly who are considered disabled according to Medicaid  Standards.The individual must meet  Medicaid  financial  requirements and Nursing Facility Level of Care  Requirements. This program is a community-based service delivered in an adult day health center which provides supervised care to adults in a supportive and safe setting during part of a day.
